MEMBERSHIP VOLUNTARY RETIREMENT:
   
Eligibility
  1. Must be a life member of I.S.A. when the husband and wife are Anesthesiologists and intend to join the scheme, they have to join separately. Upper age limit is 60 years.

  2. To promote the idieologies of I.S.A.

PROCEDURE FOR ENROLMENT
  1. Anesthetists desiring to join in the Society have to apply and obtain the application form and copy of the constitution

  2. Properly filled application form along with the admission fee/ caution deposit (to be implemented from 01/04/2010)

    Up to 35 years - Rs. 3000/-
    Up to 40 years - Rs. 4500/-
    Up to 45 years - Rs. 6000/-
    Up to 50 years - Rs. 8000/-
    Up to 55 years - Rs.10000/-
    Up to 60 years - Rs. 15000/-

  3. Xerox Copy of subscription receipt of life membership of ISA, and Proof of Age(Ex:10th class Certificate or PAN card) to be submitted to the SECRETARY of the Society.

  4. Two passport size photographs have to be submitted along with the properly filled application form. One has to be affixed to the application and one for office records.

FEES:
  1. Admission Fee/ Caution deposit as per the above table.

  2. Every member of the society shall pay Rs. 200/- (Rupees two hundred only) each time when called upon by the secretary towards fraternity contribution to the nominee of the deceased member. The member should give nominee’s name in order of preference.

  3. If the member wants to change the nomination, he is at liberty to do so by executing an affidavit on Rs. 10/- non-judicial stamp duly attested by notary. With the permission of managing committee, Secretary of Society shall then delete the original nomination and substitute the new one as per the application and it shall be deemed that it has come into force on receipt of application by the Secretary.

  4. The fraternity contribution will be drawn from the caution deposit amount of the member and paid to the nominee within 60 days of the claim. Then a demand will be made by the secretary of the Society on each member once anYear (October) for the outstanding dues on that date.

  5. Fraternity contribution bills will be sent once on 1st October every year.

  6. Rs. 50/- has to be added to each bill as administration charges.

  7. Please intimate the office, the change of address, telephone number and mobile number.

PRIVILEGES OF MEMBERS:
In the event of death of a member, his nominee is entitled to receive a lump sum amount.
Fraternity payment calculated at Rs. 200/- per member.
VOLUNTARY RETIREMENT:
In the member voluntarily retires from the membership by giving a written request to the Secretary. His/Her membership will be terminated and he/she gets the refund of his /her caution deposit amount after deducting all the dues of Fraternity contribution as on the date of retirement.
 
TERMINATION OF MEMBERSHIP:

The membership in the Society will be terminated on the following grounds.

  1. If any member furnished wrong information in his /her applications form or at any time during the period of subsistence of his /her membership by which he/she violates any provision of this society, and thus tries to obtain any unfair advantage under this society, then after giving him/her an opportunity of being heard, the managing Committee of the Society shall terminate his/her membership of the Society by passing a resolution by majority members of the managing committee.

  2. If a member fails to pay his/her share of Fraternity Contribution within 30days of the demand by the Secretary he/she shall be treated as a defaulter and he/she shall be penalty of Rs.50/-per month or part thereof as late fee along with his contribution .If a member do not clear his /her dues within 3months after the demand by the Secretary, his/her membership shall automatically stands terminated and the caution deposit shall deemed to have been forfeited.

  3. If a member at any time , for any reason ceases to be a member of I.S.A. his/her membership of this society shall automatically stands terminated .But if such a member revives his/her membership of I.S.A within a period of 6 months, he/she will get his/her membership of the Society revived on payment of Rs. 100/- and fraternity Contribution payable by him during the period he was not a member .if he/she does not get his/her membership revived with in the stipulated period, he/she will be treated as voluntarily retired member and he/she will be given such benefits as per Clause 5. Revival of membership can be done only once.

REJOINING OF MEMBER
After termination of the membership as per Clause 5 or 6, the member can rejoin in the society as a new member by paying the admission fee/caution deposit and submitting a fresh application provided he/she is not barred by age of termination under Sub-clause 3 of Clause.

The member has to obtain membership of I.S.A before applying for rejoining. Members who have rejoined as fresh members will have to pay fraternity contributions for 20 years from the date of rejoining.
 
CLAIMS: Procedure
  1. In the event of death of a member the nominee will inform the same to the Secretary of the Society and make a claim by submitting prescribed claim forms available with the Secretary/ Website.

  2. The Claim form shall be duly filled in along with the relevant certificate, and other relevant enclosures as mentioned in the claim form, are to be submitted to the Secretary of the society, duly attested and forwarded by the Secretary / President of the concerned city branch within one year from the date of the event. In case there is no city branch at that place the President / Secretary of the state branch can attest and forward the claim.

  3. The nominee or the second nominee, in case of the death of the first nominee of the deceased member will get the claim as per the preference given by the member.

  4. Computation of fraternity contribution benefit payment: The fraternity contribution is calculated according to the membership strength as on 1st day of April of that year, less dues of Fraternity contribution, if any from the member at the time of the event occurred.
